We are located inside the Sam's Club in Chino. We operate as independent doctors of optometry and therefore you do not need a membership card to see us. Just let them know at the door that you will be seeing an eye doctor.
Please bring your photo ID, your medical insurance card, and vision plan information. Please bring your glasses and contact lenses. If you lost your glasses or are out of contact lenses, you are welcome to bring a copy of your old prescription and/or bring the old contact lens boxes that you were prescribed. This will also be useful if you want to know if your prescription has changed.
Contact lens fitting services and specialty evaluations (i.e. myopia control) are not an included part of a comprehensive eye exam and additional fees apply. Fees are dependent on the type of evaluation, contacts, and complexity of the case.
Routine vision exams are covered by your vision plan. Routine eye exams only include only the following:
Any eye disease or disorder of the eye will not be covered by your vision insurance and must be billed to your MEDICAL insurance. Examples of conditions that will be billed to your MEDICAL INSURANCE include, but are not limited to the following:
Your medical office visit copay is determined by your PPO insurance plan that we are contracted under*. If you have an HMO insurance or no medical insurance, a medical office visit follow-up fee will apply.
*Currently we are contacted with Blue Shield PPO, Medicare, IEHP, Loma Linda Risk Management, and Cigna PPO.
A specific amount of time is reserved especially for you and we strongly encourage all our patients to keep their scheduled appointments. We are always happy to accommodate you, however, if you are late please note that there will likely be a wait time as scheduled appointments have priority.
We make every effort to confirm your appointment ahead of time by calling, emailing, and texting you. If we do not obtain a verbal, emailed, or texted confirmation within 72 hours of your appointment time, your appointment will be released.
Each time a patient misses an appointment without providing further notice, another patient is prevented from receiving care. Patients who do not show up for their confirmed appointment without a call to cancel at least 24 hours before the appointment will be considered a NO-SHOW. A $25.00 no-show fee will be charged to the patient for each appointment that is missed or rescheduled within 24 hours of the original appointment time. “No Show” fees will be billed to the patient. This fee is not covered by insurance and must be paid in full prior to your next appointment.
We try our best to accommodate our patients and provide a convenient time for your eye examination or emergency medical visits. If you are being seen as a walk-in, please note that already scheduled appointments have priority and the anticipated wait time is unknown and varies depending on time of day.
Yes, please! We need to know your entire list of medications, ocular history, and medical history. Understanding your medical history can better help us take care of you, your eyes, and better manage any possible eye condition that you may have.
Absolutely, however, if you are experiencing dry eye we highly recommend coming in with no eye makeup so we can better evaluate the front surface of your eyes.
